Diabetes Health Promotion Program

The Diabetes Health Promotion Program offers health promotion, prevention, screening and education focused on reducing the risk of diabetes and supporting individuals and families living with diabetes.

What is a Diabetes Health Promoter?

A Diabetes Health Promoter is trained in diabetes prevention and management and supports community members with education about risk factors, healthy lifestyle choices, early detection and diabetes self-management.

How We Can Help

  • Provide information about diabetes and associated risk factors
  • Support healthy lifestyle changes through education and community programming
  • Offer blood sugar screening and diabetic retinopathy screening
  • Coordinate referrals for foot care, nursing services and other specialized supports

Program Goals

  • Increase understanding of diabetes among clients, families and the community
  • Deliver culturally appropriate workshops focused on health promotion and prevention
  • Support individuals and families living with diabetes through guidance and education
  • Build coping skills, reduce risk factors and promote wellness
  • Provide client-centred care through screening, referrals and diabetes self-management support
